Our Services

The goal of the Aging & Disability Resource Hub is that older adults and people with disabilities of any age in Sonoma County will have access to the knowledge and option to assist them to live longer, live safely and live well in the environment of their choice.

Enhanced Information and Referral Services:

  • Information & Assistance (I&A) provides personalized assistance to older adults (60+) and people with disabilities, and their families, friends, caregivers, advocates and other representatives
  • Comprehensive information on community resources and public programs

Options Counseling:

  • Options Counseling is a collaborative process that provides guidance and empowers individuals and their families to make informed decisions about benefits, services and supports
  • Assistance in identifying and prioritizing goals and help coordinating access to public and private long-term services and resources
  • Available resources and options may include in-home care, community services, care management services, residential care, and skilled nursing care

Short-Term Service Coordination:

  • Short-term services coordination provides expedited access to services and supports for individuals at risk of institutionalization or hospitalization
  • Priority is given to consumers who need urgent access to services and programs, without which their health and safety would be at risk

Transition Services:

  • Transition Assistance helps individuals who are currently in a hospital, nursing facility, or other institution and wish to transition home or the community
  • Support includes information and access to the full range of available resources and services to support the transition process and assist with in-home care needs, including paperwork, planning and advocacy
